Family Practice of Hudson Falls, PC is proud to serve you as your nationally recognized "Patient Centered Medical Home" a team based approach to providing the best possible primary care.  We use evidence-based medicine supported by technology such as the electronic medical record.  It is our goal to improved clinical quality and provide the best health care services for your specific needs. 

As a Patient Centered Medical Home we strive to communicate effectively with our patients and provide them with the ability to take and active role in their care.  Additionally, patients are cared for by a team of highly trained professionals consisting of a Board Certified Physician, two Certified Nurse Practitioners, and Nursing and Clerical staff all trained to provide comprehensive compassionate care.

The following principles help guide our actions as a Patient Centered Medical Home Practice:
  • A clinical team is responsible for your care.
  • We work with other healthcare providers to ensure your care is coordinated and timely.
  • We provide care based on national guidelines that have been proven to be effective.
  • We work with you to set goals for your care and provide tools and techniques to help you manage your own care.
  • We have adopted Shared Medical Decision Aides to help prepare you to make informed value-base decisions with your Care Team.
  • We listen to your concerns and respond promptly to requests for clinical and medical care.
  • We provide care for all of your healthcare needs including behavioral health either directly or by coordinating with other healthcare providers.
  • We provide equal access to all patients regardless of source of payment.
  • We provide potential sources for insurance coverage to help support your healthcare needs.
We are currently welcoming all new patients to our Practice. To make this transition as easy as possible, we have created a "New Patient Packet" which is available to you by calling 518-747-4117.  It is our goal to provide you with quality care from the very beginning.  To do this, it is important for your new Provider to know your current status. The first step in achieving this, is to request any previous medical records.  A Medical Record Release Form may be found on this website or you may call our office and speak to our Medical Records Clerk, Becky and she will be happy to help you with this process.

We offer convenient hours for your medical care, including early morning and evening appointments.  Our Practice also reserves time for same-day appointments for routine and urgent care. Please call (518) 747-4117 to make your appointment.

Please call to cancel or reschedule your appointment at least 24 hours in advance, so we may offer that time to another patient in need.  If you do not call to cancel or reschedule within this time period you may be subject to a $25.00 "no show"  or cancellation fee.

If a patient, no shows three times, we reserve the right to dismiss that patient.

If you have a medical emergency, call 911 or go to the nearest emergency room.

Should you require a hospital stay, the Hospitalist will provide the attention you need 24 hours a day, 7 days a week. At discharge, the Hospitalist will send a discharge summary and arrange follow-up care with our Practice.

Family Practice of Hudson Falls, PC utilizes Surescripts for e-prescribing, a web based solution that exchanges prescription information electronically with your pharmacy. The system interfaces with our practice management system which means updated patient information is linked to our prescriptions. This is especially important in providing accurate medication lists for our patients and to ensure proper drug utilization review of dosing and drug interactions to enhance patient safety. There also is an interface to Rx Hub which links us to many mail order pharmacies and allows the prescription to be checked for insurance formulary compliance.

Requests for new medications (including antibiotics) will not be taken over the phone or through the website. For the safety and well being of our patients, an appointment must be scheduled so that an evaluation by a Provider may be completed.

As of August 27, 2013, The New York State Prescription Monitoring Program (PMP) requires controlled medication verification before prescribing.

Family Practice of Hudson Falls, PC does not offer chronic pain management and will not dispense chronic pain medication. We will provide you with a referral to a pain management center if you need this specialized form of care after evaluation by our Providers.

Please access our Patient Portal or call your pharmacy to request most non-urgent, non-controlled medication refills.  Please allow 3 days for refills to be processed.
Family Practice of Hudson Falls, PC is a private physician group practice.  We understand that patients do not plan to be ill and health care costs may cause financial hardship for patients and their families.   We are dedicated to working with our patients to arrange payment plans when necessary.

All patients are responsible for paying their co-pay at the time service is rendered and for paying any balance due after insurance immediately after receiving their first statement.

For your convenience, we accept cash, checks, or credit cards.

There is  a $50.00 fee for all returned checks.

Our practice will file insurance claims on behalf of patients and will balance bill patients after the insurer has either paid or denied the claim.  When an insurance claim is denied, we will make a reasonable effort to appeal those decisions when appropriate.

Except as limited by law (i.e. Medicare or Medicaid patients), or by contract (some HMO or PPO insurers), any service fee not paid by an insurer is the responsibility of the patient.

Patients who do not make reasonable progress toward paying outstanding obligations may be released from our practice.  Furthermore, the practice may refer the account to a collection agency and the debt will be reported to credit agencies. A $25.00 service fee is added to all accounts referred to collections.

Patients terminated from the Practice due to outstanding obligations will be given thirty (30) days notice during which time they will be followed for emergency medical care services only. Patients are still financially responsible for paying for any services rendered during this time.
